With a Body Condition Score of 2/9 your horse is very skinny and it is wise to contact your vet first. He or she can see whether a health issue is a reason for this. In the meantime, try to give your horse as much grazing and roughage as possible. If your horse cannot eat 'normal' roughage, such as hay and grass, take a look at roughage substitutes that you can also feed soaked.
